Program Overview

Practical, daily-living support that meets people where they are.

Our Rehabilitative & Community Support Services (RCSS) program partners with individuals, families, and care teams to deliver structured, goal-driven support in real-world settings. We work alongside the people you love to translate clinical recommendations into everyday wins — better routines, stronger relationships, and a clear path forward.

Every plan is built around the individual: their strengths, preferences, culture, and goals. Services are flexible, trauma-informed, and grounded in evidence-based practice.

Adults and youth we support through RCSS

Who we serve

Adults and youth living with mental health conditions, developmental needs, or recovery-stage challenges who benefit from hands-on community-based support to maintain stability and pursue meaningful goals.

  • Adults with serious and persistent mental illness
  • Youth transitioning to greater independence
  • Individuals stepping down from higher levels of care
  • Clients with co-occurring needs

What's Included

Skill-building, coaching, and connection — wrapped around each client.

Our team blends one-on-one coaching with community engagement so progress translates directly into the spaces where life happens.

Daily Living Skills

Hands-on coaching in personal care, household management, time management, transportation, and money skills.

Social & Community Integration

Support to build healthy relationships, access community resources, and participate in social, recreational, and civic life.

Symptom Self-Management

Practical coping tools, relapse-prevention planning, and reinforcement of treatment goals from the broader care team.

Independent Living Readiness

Skill-building to support safe, stable, community-based living — including housing navigation and home-safety routines.

Family & Caregiver Coaching

We equip the people around the client with practical strategies, language, and tools to reinforce progress at home.

Flexible Scheduling

Sessions are scheduled around the client's life — at home, school, work, or community settings during hours that fit.

How It Works

A clear, collaborative path — from referral to results.

We make it easy for clients, families, and referring providers to engage. From the first call to ongoing care, every step is guided, documented, and built around measurable goals.

  • Referral received and reviewed within one business day
  • Intake and strengths-based assessment scheduled
  • Individualized service plan co-designed with client and family
  • Regular in-community sessions with a dedicated team
  • Ongoing care coordination with treatment team and providers
  • Progress reviewed and plan updated on a defined cadence
